New Patient Information

Please click on this Patient Intake Document link, print and fill out the New Patient intake pages, and bring them with you to your initial visit.

Please bring in with you the nutritional supplements and medications that you are presently taking.

Many of my patients suffer from environmental sensitivities and can’t tolerate any perfumes or other scents, so please refrain from using any scented hair or body care products on the day of your visit.  The same goes for tobacco scent.  I will need to reschedule anyone who arrives smelling of chemical scents or tobacco.

Also, due to the compromised immune systems of many of my patients, I try to keep my office free of infectious agents.  The scope of my practice does not include acute infections.  Please call to reschedule if you have a cold or the flu.

If you are planning to use your insurance, please confirm with your insurance company that your particular policy covers office visits to naturopathic physicians, and confirm the benefits allowed.   I am a preferred provider for several health insurances, including: Regence Blue Shield/Uniform Medical Plan, Premera Blue Cross, LifeWise of WA,  North Star Administrators, Northwest Admistrators, Moda Health, some of the First Choice Health companies and Great-West Healthcare.

We request that any office visit reschedules or cancellations be made at least 24 hours in advance of the booked appointment.
